Please, read the following registration and attendance rules for NCADD:

1. All participants need to complete the annual Registration Forms. These forms will be needed for the Valentine Dance. Once completed these forms will serve for the whole year.

2. All minors and conserved participants will need the signature of their parent or conservator/guardian on these Registration Forms. No other individuals can sign these forms.

3. All minors and conserved participants will need to be supervised by a responsible adult,18 years or older, for the entire NCADD program/event.

4. Adult participants who are independent can attend alone. However, if the participant has any behavioral issues, we ask that a responsible adult,18 years or older, supervise the participant for the entire NCADD program/event.
